Relocating to the Florida Panhandle is an exciting transition, but setting up new healthcare can be a hurdle. We’ve designed our onboarding process to be as smooth as possible for our service members and their families.
[ ] Update TRICARE Enrollment: Ensure your region is updated to TRICARE East so your coverage is active the moment you arrive.
[ ] Tyndall, NSA & USCG Families: We prioritize "Establishment of Care" visits for new arrivals to ensure there is no gap in your chronic medication refills or preventative screenings.
[ ] Record Transfer: You can have your previous MTF or civilian provider fax your records directly to us at (850) 702-5704.
[ ] School & Sports Physicals: New to the area and need forms for Bay County Schools? We offer quick turnarounds for student-athletes and new enrollments.
[ ] Schedule Your 'Establishment of Care' Visit: Don’t wait until you are ill. Schedule a wellness visit to establish your medical home in the Panhandle.
[ ] Medications: Bring a list of your current prescriptions to your first visit to ensure uninterrupted refills.
